Fluoride has been the default ingredient in toothpaste for over 60 years. It is backed by decades of clinical research, endorsed by every major dental association, and used by the vast majority of dentists worldwide. That track record is real.
But a growing number of people are looking for alternatives. Some want to avoid fluoride for their kids. Others are concerned about long-term ingestion. And a newer ingredient, hydroxyapatite, is showing up in more and more toothpaste brands with claims that it can do everything fluoride does without the risks.
So what is hydroxyapatite toothpaste, and is it actually a legitimate replacement for fluoride? The short answer: the clinical evidence is stronger than most people expect.
What Is Hydroxyapatite?
Hydroxyapatite is a form of calcium phosphate. It is the same mineral that makes up roughly 97% of your tooth enamel and 70% of your bone structure. When used in toothpaste (typically as nano-hydroxyapatite or n-HA), it works by physically bonding to the surface of your teeth, filling in microscopic cracks and weak spots in the enamel.
This is different from how fluoride works. Fluoride converts the surface layer of your enamel into a harder, more acid-resistant material called fluorapatite. It changes the chemistry of the enamel surface. Hydroxyapatite, by contrast, restores enamel using the same mineral your teeth are already made of. It fills in rather than coats over.
Hydroxyapatite has been used in Japanese toothpaste since the 1980s. It is approved as an anti-cavity ingredient by the Japanese government and has been a standard option in Japanese oral care for over 40 years. Its adoption in the US and Europe is more recent, but it is accelerating quickly.
What the Clinical Research Shows
Here is where it gets interesting. Multiple randomized clinical trials have now compared hydroxyapatite toothpaste directly against fluoride toothpaste for cavity prevention and enamel remineralization.
The 2023 adult trial (18 months, double-blind, randomized): Published in Frontiers in Public Health, this study compared 10% hydroxyapatite toothpaste against fluoride toothpaste in adults. The result: hydroxyapatite was statistically non-inferior to fluoride. 89.29% of subjects in the hydroxyapatite group had no increase in cavities, compared to 87.36% in the fluoride group.
The children's trial (published in BDJ Open): Compared hydroxyapatite to 500 ppm amine fluoride toothpaste in children. Researchers found no statistically significant difference in remineralization or cavity prevention between the two groups.
The 2024 systematic review and meta-analysis (ScienceDirect): Pooled data from multiple trials and concluded that hydroxyapatite toothpaste provides "non-inferior" caries prevention compared to fluoride toothpaste.
"Non-inferior" is scientific language for "works just as well." Not better, not worse. Equivalent.
Where Hydroxyapatite Has an Edge
While the cavity prevention data shows the two are roughly equal, hydroxyapatite has some advantages that fluoride does not:
Safe if swallowed. This is the big one for parents. Fluoride in small doses is safe, but children who swallow too much fluoride toothpaste during the years their teeth are developing can develop dental fluorosis (permanent white spots on the enamel). Hydroxyapatite carries no such risk. It is a biocompatible mineral that your body already produces.
Reduces tooth sensitivity. Multiple studies have found that hydroxyapatite toothpaste reduces hypersensitivity more effectively than fluoride toothpaste. The mineral fills in exposed tubules in the dentin, which is what causes that sharp pain when you eat something cold or acidic.
No toxicity concerns at any dose. Fluoride is safe at recommended doses, but it does have a toxicity threshold. This is why fluoride toothpaste carries a warning label for children under six. Hydroxyapatite does not have a known toxicity threshold when used in toothpaste.
Microbiome friendly. Fluoride works partly by inhibiting bacteria. Hydroxyapatite works by strengthening the tooth surface itself. Some researchers believe this is a more targeted approach that does not disrupt the broader oral microbiome.
Where Fluoride Still Wins
Fluoride has legitimate advantages that are worth acknowledging:
More research. Fluoride has been studied for over 70 years. The body of evidence is enormous. Hydroxyapatite research is growing quickly, but the total volume of studies is still smaller.
FDA recognition. The FDA has approved fluoride as an anti-cavity agent. Hydroxyapatite has not received FDA approval for that claim in the US. No hydroxyapatite toothpaste carries the ADA Seal of Acceptance. This does not mean it does not work. It means the regulatory process has not caught up with the clinical data.
Cost. Fluoride toothpaste costs $1 to $6 per tube. Hydroxyapatite toothpaste typically runs $8 to $20 per tube. The price gap is narrowing as more brands enter the market, but fluoride is still cheaper.
Availability. You can find fluoride toothpaste in every grocery store, gas station, and pharmacy in the country. Hydroxyapatite options are expanding but are still more of a specialty purchase.
What to Look for in a Hydroxyapatite Toothpaste
Not all hydroxyapatite toothpastes are equal. If you decide to make the switch, here is what matters:
Concentration matters. Clinical trials showing effectiveness used 10% hydroxyapatite concentration. Some brands use lower concentrations that may not deliver the same results. Check whether the brand discloses its percentage.
Nano vs. micro. Nano-hydroxyapatite (n-HA) is the form used in clinical research and is more effective at penetrating enamel surfaces. Look for "nano-hydroxyapatite" specifically on the ingredient list.
Skip the junk. Switching to hydroxyapatite toothpaste does not help much if the rest of the formula is loaded with artificial sweeteners, titanium dioxide, SLS, or synthetic dyes. The whole point is a cleaner product.
